Why do the two guidelines disagree?
The American thresholds moved down in 2017, so 130/80 is stage 1 hypertension there. European practice still calls 130-139 over 85-89 high normal and starts hypertension at 140/90. The reading has not changed; only the line drawn through it has. Both are shown so a result is not made to look worse or better than your own health service considers it.
Why average several readings, and why drop the first?
Because a single reading is a snapshot that moves with the time of day, a conversation, a full bladder and the walk to the chair. The first reading is reliably the highest — the cuff inflating is itself a small stressor — which is why guidelines advise taking two or three a minute apart and averaging the later ones.
What counts as urgent?
A reading at or above 180/120 is treated as urgent by health services. Sit quietly for five minutes and measure again; if it stays there, or if there is chest pain, breathlessness, weakness, difficulty speaking or a severe headache, contact emergency services rather than waiting. That is a safety message, not a diagnosis.
Is a high reading a diagnosis of hypertension?
No. Hypertension is diagnosed on repeated measurements over time, usually with home or 24-hour monitoring, because clinic readings capture white-coat hypertension and miss its opposite, masked hypertension. Nothing on this page is a reason to start, stop or change any treatment.
Why does my home monitor read lower than the surgery?
Partly because you are calmer, and partly by design: home thresholds are set lower to match. 135/85 measured at home is treated as equivalent to 140/90 in a clinic. Arm position matters too — an arm hanging by your side rather than resting at heart level reads about 10 mmHg high.
